1. Keep the following information/documents ready before you begin to fill the online Registration Form as these are MANDATORY fields.

• a) Proof of Age : Birth certificate of the child in pdf format. Please note that the Date of Birth, Name of the Applicant, Father’s and Mother’s Name filled in the form must match with those recorded in the Birth Certificate.

• b) Proof of residence:

  • Ration Card issued in the name of parents
  • Domicile Certificate of child or of his/her parents.
  • Voter I-Card (EPIC) of any of the parents.
  • Electricity Bill/MTNL telephone bill/Water bill/Passport in the name of any of the parents or child.
  • Aadhaar Card/UID Card issued in the name of any of the parents or child.
  
  (above mentioned documents should be uploaded in pdf format)

  For Distance Point calculation, it is mandatory to choose the location from the options available in the registration form. Distance will be calculated and cross-checked with the proof of address submitted/ mentioned.

• c) Proof of Sibling – Report Card/I-Card of the real brother/sister studying in MAPS (pdf format).

• d) Proof of Alumni – X / XII CBSE Exam Certificate (pdf format)

• e) Photograph of the child -
  Recent passport size photo of the child (in jpg/jpeg format).
  The digital size of photo must be less than 50 KB.
  Background of the photo should be white.

• f) Valid email id and mobile number of either of the parent should be provide.

2. In case any information is found to be false/incorrect/incomplete the school reserves the right to reject the registration form.

3. Please Print & retain the acknowledgement receipt generated by the system once the form is submitted successfully. Parents are not required to submit the hardcopy of the downloaded registration form at the school.

4. On successful submission of the online Registration Form, parents will receive the Username & Password as SMS on their registered mobile number. You are requested to retain the same for future login in maps.campuscare.cloud

5. School will not be responsible for any technical error that may occur while filling up the online nursery registration form.

6. Please note that the successful submission of the application and acknowledgement does not guarantee admission.
